CSB Releases Final Report on 2015 Torrance Refinery Explosion

An independent federal agency that investigates chemical accidents, the U.S. Chemical Safety Board, released its final report on the investigation of the 2015 explosion at the ExxonMobil refinery.  There were several disturbing conclusions mentioned.  Among them:

  • Multiple instances of where the operators violated ExxonMobil’s corporate safety standards
  • A tank containing large amounts of modified hydrofluoric acid (MHF) was narrowly missed by flying debris.  If the tank had been hit, it’s possible that MHF, a highly toxic material would have been released.
  • The CSB has requested more information from ExxonMobil regarding safeguards against release of MHF, and has received no response.
